1. 7352-Isala Meppel Hospital Discharge Information

Your attending physician has discussed with you that your treatment is complete and that you can leave the hospital. In this folder we give you some advice about your aftercare at home. During a personal interview the nurse will fill in the information that is important for you.

Your family doctor

Your attending specialist will inform your family doctor about your hospital admission, any medication and aftercare. In this way your family doctor will be aware that you have returned home, so that he can assist you further. You will also receive this discharge summary to take home with you.
