​Your attending physician has discussed with you that your treatment is complete and that you can leave the hospital. In this folder we give you some advice about your aftercare at home. During a personal interview the nurse will fill in the information that is important for you.

Your family doctor

Your attending specialist will inform your family doctor about your hospital admission, any medication and aftercare. In this way your family doctor will be aware that you have returned home, so that he can assist you further. You will also receive this discharge summary to take home with you.

Medication needed

O  Yes, your pharmacy has received a prescription from us. You can collect your medication from your own pharmacy after 4 p.m.
O  Yes, take the prescription to the pharmacy yourself.
O  If necessary, take two 500 mg paracetamol tablets four to six times a day in consultation with the nurse.
O  No.

Please note

  • Paracetamol is not a prescription medicine and has to be paid for.
  • Plasters also have to be paid for.

Contact

In the event of complications arising from your admission you can ring the outpatient clinic of your attending physician between 8 a.m. and 4.30 p.m.

After 4.30 p.m. and at weekends you can contact the Emergency department, telephone number +31 522233268.

Your attending physician will remain your first point of contact up to and including 10 days after discharge. After that your family doctor will be your first point of contact, unless otherwise arranged with you by the physician.
