You will have an operation soon. Before the day of your operation, you will have a telephone appointment with the anaesthetist. You will also have a telephone appointment with the nurse. If you are taking any medicines, you will also have a conversation with the pharmacy assistant. These appointments all form part of your preoperative screening. We would appreciate it if you could prepare for these appointments at home. This brochure tells you more about how to do so.
What happens during your preoperative screening?
Before you are admitted, we will carry out a preoperative screening. This involves checks and a risk assessment in relation to your health and your medical details. We will ask you to complete a digital questionnaire before you are admitted. Next, you will have an appointment with the pharmacy assistant to discuss your medication and any allergies.
The nurse will assess your health and care requirements during and after admission based on the questionnaire you filled in. The anaesthetist will check you records and assess the complications of your local or general anaesthetic, for example. In some cases, additional tests may be required.
Once the nurse and anaesthetist have given the go-ahead, your admission date will be scheduled.
Your telephone appointments
Where to find information about your appointments:
- The dates and times can be seen in MijnIsala under Appointments (Afspraken). MijnIsala is a secure website for Isala patients. It is available 24 hours a day, 7 days a week.
- We will call you around the time listed for your appointment. We will do so using a withheld number.
In some cases, your doctor may need you to come to the hospital instead.
- This depends on your health and the time and the type of operation you will have.
- If you would prefer to attend an appointment at the hospital instead, because you have hearing difficulties or you struggle with (Dutch) language, please contact the Preoperative Screening department. The same applies if you wish to change an appointment.
Preparation
How to prepare for your appointments at home.
- To ensure we can safely put you under anaesthetic, we need information about your health. You will be asked to fill in a questionnaire for that purpose. This brochure provides more information about that questionnaire.
- Please keep the right information to hand during your appointments. More details on what you need can be found under 'What do you need during your appointments?'.
Questionnaire
The questionnaire contains questions about your health. Please fill in all of the questionnaire, before attending your appointment. If you do not fill in the questionnaire, we will not be able to go ahead with your appointment.
Complete the questionnaire using MijnIsala
You can complete the questionnaire at home using MijnIsala.nl
You can log in using one of two methods:
- Log in to MijnIsala.nl using your DigiD and confirm your login using the extra text message verification.
- Log in using the DigiD App.
Don't have DigiD?
If you don't have DigiD, you can request it. Please allow for the fact that it can take a few working days to activate your DigiD.
Complete the questionnaire at the hospital
You can also complete the questionnaire on a hospital computer. You will need a numerical code to do so. Please call the Preoperative Screening department on +31 88 624 21 39 to obtain your code.
How to complete the questionnaire
- Go to Questionnaires (Vragenlijsten) via MijnIsala. It will take around 20 to 30 minutes to complete the questionnaire.
- Please note that you'll need to complete the questionnaire in one go, as you will not be able to save your progress.
- If you are unable to complete the questionnaire via MijnIsala, simply do so using one of the computers at the hospital.
Have you shared an up-to-date overview of your medication with Isala?
- If not, you simply need to give consent for the details of your medication to be shared. You can do so via your own pharmacy (by telephone or at the counter) or via volgjezorg.nl.
- If you prefer not to give consent for your details to be shared online, simply request a written overview of your medication from your pharmacy.
What will we be discussing with you?
Good to know
The staff at OT Planning (for the specialism that will carry out your operation) will notify you of the date of your operation. They will call you or e-mail you to let you know, or you will receive a letter at your home address. In other words, the anaesthetist, nurse or pharmacy assistant will not be able to tell you the date of your operation.
Your conversation with the pharmacy assistant
You will only have a conversation with the pharmacy assistant if you are taking medication.
- The pharmacy assistant will discuss your medication with you.
- The pharmacy assistant will ask you whether you have any allergies and/or serious side effects from your medication.
- If anything changes in the medication you are taking, please call the pharmacy assistant on +31 88 624 21 39. The pharmacy assistant will then note this change in your medical record.
If you are taking any medication you have bought from the chemist or pharmacy yourself, such as paracetamol, vitamins or antacids, please tell the pharmacy assistant about this during your conversation.
What will we be discussing with you?
Your conversation with the anaesthetist
The anaesthetist will administer anaesthesia during your operation. On top of that, they will monitor your heart rate, blood pressure and breathing, and make sure you don't feel any pain.
The anaesthetist you speak to during your preoperative screening is usually not the same anaesthetist who will be with you on the day of your operation.
- You will discuss your health with the anaesthetist.
- The anaesthetist will decide which medication you can and cannot take around the date of your operation.
- The anaesthetist will also inform you about the anaesthetic that will be used.
- The brochure you receive on your anaesthetic will be uploaded to MijnIsala after your conversation with the anaesthetist.

Is your child having an operation?
- Children up to the age of 12 do not need to be present during the conversation with the anaesthetist.
- Children between the ages of 12 and 16 do need to be present during the conversation with the anaesthetist.
- Children over the age of 16 will have a conversation with the anaesthetist themselves. You can be present during this conversation as a parent or caregiver.
What will we be discussing with you?
Your conversation with the nurse
The nurse will discuss the agreements made to prepare for your operation and for your aftercare. On top of that:
- the nurse will run through a number of questions that are important to the nursing department. This will help us find out which symptoms or health problems you have, and how these affect your daily life. We would also like to know what you would like to see in terms of support from other people.
- the nurse will discuss your home situation with you.
- the nurse will discuss whether you need any support at home. If so, they will tell you which organisation you can contact to arrange this.

What do you need during your appointments?
- The completed questionnaire about your health.
- An overview of the medication you are taking. Alternatively, simply keep the packets your medication comes in to hand.
- An overview of any medication you buy yourself at the chemist or pharmacy.
- If you are using a Baxter roll: the Baxter dispensing list.
- The names and phone numbers of two contacts.
